About Uttar Preadesh Pharmacy Council:

UTTAR PRADESH Pharmacy Council is a statutory body constituted by the Government of Uttar Pradesh under the provisions of the Pharmacy Act 1948, in the year 1956 through UP Govt GAZETE notification. The main objective of the UP Pharmacy Council is to regulate the practice of pharmacy in the state of Uttar Pradesh.

Online Registration Forms:

Registration, Renewal, Transfer and Related Online Services of Uttar Pradesh Pharmacy Council

To go hand-in-hand with technology, Uttar Pradesh Pharmacy Council has automated its various services for the ease and good of Pharmacists, which were running offline previously. This portal facilitates pharmacists to get themselves registered with UPPC to avail its online services. To register, pharmacists need to fill their basic details and get their Mobile No./Email ID verified through One Time Password. After registration on portal, pharmacists can login and submit application for any of the following services for which they are eligible:

  • Request for Pharmacist Registration
  • Request for Renewal/Restoration of Pharmacist’s Registration
  • Request for Transfer of Pharmacist from other Council to UPPC
  • Request for NOC for Transfer from UPPC to other Council
  • Request for Duplicate Certificate/Renewal Card Generation
  • Request for Good Standing Certificate
  • Request for Change/Upgradation in Qualification
  • Request to Modify Name and Address

User can also submit the Application Fee through the portal. Once the fee is submitted, application is forwarded on UPPC Login. Concerned authority scrutinizes the application and Approves/Rejects/Raises Query against the Application.

Applicant will have to redress/answer the query, if any, raised by UPPC at the earliest.

Applicant can also:

  • Track the Status of Submitted Application
  • Download the Submitted Application and Fee Payment Receipt
  • Recover the password in case he/she forgets password
  • Schedule appointment for Document Verification
  • Apply for multiple services at a time
  • Partially submit the application, and remaining part later
  • Edit the filled details until fee is paid
How To Register?
Step 1 - Registration:

Open http://apps.uppc.in/online/Account/Registration in your web browser and fill Title, First Name, Middle Name, Last Name, Email ID & Mobile No. in respective field and click on Register button.

Thereafter, a One Time Password (OTP) will be sent on the Mobile No. & Email ID entered by the applicant. Verify that OTP by submitting on next step and then create the desired Login Password. 

Once the password is created, registration will be completed. 


Step 2 - Login:

Open http://apps.uppc.in/online/ in your web browser and fill Registered Mobile No. & Password in respective fields and click on Login button. 

After login, user will be redirected to his/her respective dashboard. 

In case user forgets password, click on Forgot Password? link and get the registered Mobile No. verified through OTP. Thereafter, new password will be sent on the registered Mobile No. & Email ID.

DOCUMENTS REQUIRED FOR VERIFICATION PROCESS 

1. FOR FIRST REGISTRATION 

1. Marksheet of High school.

2. Marksheet of Intermediate. 

3. Marksheet of Diploma/Degree in Pharmacy of all the years 

4. Certificate of High school. 

5. Certificate of Intermediate. 

6. Diploma / Degree of Pharmacy or Provisional certificate issued from the college showing passed Diploma/Degree in pharmacy examination 

7. Practical Training certificate of 500 hours from PCI recognized Government Semi Government and Government aided Hospitals for Diploma Pharmacy candidate only. 

8. Current Domicile Certificate (issued within six months) for those candidate who are passed from outside of UP. 

9. Six Colored Passport Size Photographs.

Note: Kindly bring original copy of all documents for verification.

2. FOR TRANSFER CASES

1. Marksheet of High school.

2. Marksheet of Intermediate.

3. Mark sheet of Diploma/Degree in Pharmacy of all the years

4. Certificate of High school.

5. Certificate of Intermediate.

6. Diploma / Degree of Pharmacy or Provisional certificate issued from the college showing passed Diploma/Degree in pharmacy examination

7. Practical Training certificate of 500 hours from PCI recognized Government Semi Government and Government aided Hospitals for Diploma Pharmacy candidate only.

8. Current Domicile Certificate (issued within six month) For those candidate who are passed from outside of UP.

10. U.P. State Domiciles Certificate (issued within six month)

11. Registration Certificate and Renewal Certificate (in original) issued by other state's pharmacy council where pharmacists is currently registered
